P0266 a few seconds after start - will not go!

Hi I have a DW12RUD engine and I’m getting P0266 on cylinder 2 a few seconds after start with accompanying 1500rpm idle. I believe it’s doing that because it shuts off cylinder 2 as the other 3 injectors go to 1.00mg flow.
I have replaced the injector and also coded it in.
I have checked the wiring.
I have checked the camshaft, valves and cylinder with a bore scope but to no avail.
Interestingly, if you start the van and immediately give the throttle some revs, the code won’t show until you let it idle for 3-5 seconds.
I’m absolutely losing the will to live at this point, so if anyone has any suggestions I am all ears. Thanks
