3008 Diesel Injector coding (Solved)

Okay so I'm looking for a bit of advice on this.
Injector 1 went a few weeks back, got a remanufactured injector and had it fitted.
The backstory:
Since I picked the car up, it has occasionally knocked and has been using more fuel than normal. This has been getting progressively worse, until yesterday I got to the end of my road (maybe 20m) and the ESP light came on, the knocking was constant and loud, and you could really smell diesel. Quickly got back on my drive and popped the bonnet to find a thin stream of pure white smoke coming from the injectors, and a crazy amount of heat.
Now I see this as the Injector putting out too much diesel. I believe it may be improperly coded.
I've been going to the garage where it was done for a decade and have never had an issue before, but with this they refuse to even entertain the possibility they might have made a mistake as "we've done loads for years".

When fitted, they used the code on the injector, and then did the bit where it goes to X speed and Y RPM etc.
BUT, as it is remanufactured, it came with paperwork and a new code (grading data), and even with that second part, if the initial grading code was wrong, it's all wrong?
I may well be wrong myself, but I thought that the grading code was the base level it goes from, then is calibrated with the speed/rev bit. So if the base level is wrong...

When you say injector 1 'went' a few weeks ago, how did you know and why replace it? 
I used to be indecisive, now I'm not so sure!

Sorry, I meant how did you detect a problem before scanning it to identify the problem, what was happening or was it just the limp mode?

And when you say Injector 1, are you taking into account the fact that PSA engines number from the gearbox end of the engine?
As I get older I think a lot about the hereafter - I go into a room and then wonder what I'm here after.

I meant originally, whether you felt anything wrong or smelt diesel or saw smoke or was it just the limp mode that caused you to scan it?

If you have smoke at the injectors then that sounds like a leak of diesel or a leak where an injector joins the head but then you might also get a chuffing noise.

Any smoke at injectors is a diesel leak problem ,nothing to do with the injector " putting out too much" ! It's either leaking diesel from pipes onto engine which is heating up giving white "smoke" vapor, or the injector seal is leaking ,giving combustion fumes blowing out of the injector seating ,,,,,if an injector is putting too much diesel into the cylinder you get knocking noise and white smoke out of exhaust, this is usually caused by worn components within the injectors and is not likely to be caused by injector coding, a new injector is learned by ecu during driving coding it speeds up the process, I suggest you do a diesel leak off test to see how much diesel is being leaked back and if there is excessive air in the leak off. Google diesel leak off/leak back and you will find out how to do it ,it's pretty straight forward if you buy kit off ebay ,or you can make your own with small plastic bottles and aquarium pipe

The leak off test will determine if any injectors are knackered

Okay so; heard back from the remanufacture business, most of the issues can definitely be the result of the garage using the wrong code.
However, I had recovery come out to take the car from mine to the garage, and they had a quick look first. The garage managed to tighten the holding bolt far too much cracking it, and yesterday it gave completely; half of it was gone. The injector had been loose so fuel was pushing out past, and now was gushing out.
