This will only be interesting to people who find this kind of thing interesting, but here's an article from IOSH magazine about a recent investigation where a car fell off of a two post lift in London and injured a customer who happened to be lurking underneath. I think it goes under the heading "A learning experience"

In Egypt we had one lance corporal who was noted as a bit of a 'brown nose' around the senior officers, he did just what the woman in the video did whilst talking over his shoulder to an officer.
