Berlingo 1.9D dw8 losing water from the head

Hi, Ithe car in question is a partner, but it has the same engine 1.9D ( dw8 engine ) its losing water out of an unused bolt hole on the top front face of the head. Now have I forgotten a bolt during reassembly or is this a tiny blanked of machining channel that has a tiny core plug or similar or has the head cracked because it froze ( no antifreeze ).

the lug with the bolt is btween the middle two injectors right behind the EGR..
