Xantia Heater Fan - Relay mod Instructions

My Xantia started to display the issue of the heater blower not working this morning.

Aware there is a relay mod to fix this issue, but have done a search and cant seem to find a definative set of instructions.

If anyone has done this mod, would they be able to point me in the direction of what they followed to get this working again.

(had to pull over twice to stop and start the car, just to get the blower working)
