Okay - so I've been thinking this over a bit. It it possible that theres a nut on the fuel filter for water extraction. Could that be an issue - if its on. Or - is it possible when the fuel filter was last fitted - an o-ring was omitted?

If you are referring to the water in diesel sensor (item (5) below, your vehicle was not fitted with one.
These were generally only fitted to vehicles intended for hot countries. Water is bled during normal servicing operations.
