405 STDT Engine judder / lurch

I have just bought a 405 STDT estate with 145,000 miles on it. It is rather difficult to drive smoothly. Any harsh movement of the clutch results in lurching, at low speed (25mph) in fourth the engine starts to lurch and I have to dip the clutch to stop it. Rolling off the throttle and back on causes the car to lurch and accelerating round roundabouts causes a juddering through the gear lever and steering wheel. I suspect engine mounts can anyone confirm this (and id so how difficult are they to change) or suggest other possibiltys I would be grateful.
