Mine was older but based on a 98 model:

Likely to be in the fusebox behind the glovebox (ease the side tabs in, drop box down and out, remove panel behind)

From memory it will be #16 or thereabouts, which has 2 positions, a 2 tab fuse with 3 slots so it can go left or right pair. In one of them the cig lighter socket is on only with ignition, in the other its always on - useful when camping or to trickle charge through rear socket.

Rear and front lighter were on same circuit, I used to leave the rear one out because if you load the boot you can and up pushing and holding it in - bad!

Yes it's in the fusebox behind the glovebox. The fuse can be inserted in either of two positions. The cigarette lighter can either be permanently live or controlled by the accessories position on the ignition switch. I should have the handbook for this model. If I find it I can scan whatever pages you need.
