My fans come in at when the temp gauge has passed over 90, I usually have them running most days as the result of the appalling traffic on the M4, once the car has sat ticking over for a few minutes they kick in. As mine are quite noisy theres no mistaking they've kicked in, and I breathe a sigh of relief as the guage drops!!
They also continue to run if the engine is switched off, this is quite rare and is usually the result of being stuck in traffic on a hot day, then pulling over for a break or fuel. As I am quite obsessed by cooling my faithful old XUD9TE I usually jump out and open the bonnet at this point for extra cooling!!!
I'm also quite obsessive about keeping the connections to the thermo switch clean, and also check the rad all over for uniform heat every time I service the car.
But I do spend a lot of time in Motorway jams and never want to sit there and watch the guage rise and rise with no fans!!
